- Week 10 CORE Entries Assignment CORE ENTRIES (Clinical Log) Please read and follow the Student Practicum Expectations with CORE . Students should also read and reference the Student Handbooks, all course materials, course announcements, and any documents provided by the faculty during their practicum course for information concerning practicum requirements and/or updates from faculty. CORE Entries are due by the end of day on day 7 each week*. Encounters (case logs) for all patients that you see each week must be entered within 7 days of the occurrence in the clinical documentation system (CORE ELMS) or they will not count and you will have to repeat those hours. *If you have a clinical practicum that goes to the end of day on day 7 of any week, you may enter that day’s hours by end of day of the following week. All case logs must contain at minimum of a short episodic note. The patient encounter used in the complete SOAP note should have the complete SOAP note documented in the case log. Your course faculty will review your submitted cases weekly, providing feedback and approving/disapproving your logs. Students must complete 180 direct patient contact hours for the AGNP program for each practicum course Clinical hours must be documented and will be verified by faculty at the end of the session. - Week 10 SOAP Note Assignment SOAP NOTE Instructions: Each week, you are required to submit one SOAP note using the SOAP note template. The SOAP can be for a comprehensive visit or a problem focused visit. The diagnosis for your patient should be related to the body systems covered in class work this week. Every component in the template must be filled in regardless of which type of visit you do (comprehensive vs problem focused). When doing a problem focused visit you will be documenting on pertinent findings, and pertinent negatives only. For any topic area in the template that does not apply to your patient encounter, put in the phrase “Not completed in this focused visit.” The completed note should be submitted to the Submissions Area Submission Details: By the due date assigned, enter your patient encounters into the SU clinical tracking system and complete at least one SOAP note in the template provided.
